• I understand that submission of this form does not guarantee partnership or inclusion in the Ur Massage Therapist network, regardless of the completeness or quality of submitted materials.
• I certify that all information provided is true, complete, and accurate to the best of my knowledge.
• I agree to honor all Ur Massage Therapist pricing policies for chair massage services as outlined on the official website and within booking agreements.
• I agree that any full-body massage pricing I quote to clients onsite will be honored as quoted.
• I understand that I am an independent contractor and business owner, not an employee, partner, or agent of Ur Massage Therapist.
• I acknowledge that I am solely responsible for collecting and managing all payments I receive from clients, unless otherwise indicated in advance by Ur Massage Therapist.
• In cases where Ur Massage Therapist collects full or partial payment in advance (e.g., chair massage bookings), I understand that Ur Massage Therapist reserves the right to distribute those funds either before or after the event at their discretion.
• I understand and agree that administration fees collected by Ur Massage Therapist are non-transferable and not owed to me under any circumstances.
• I agree to provide and transport my own massage chair, massage table, linens, supplies, and tools to every scheduled event or appointment. Ur Massage Therapist is not responsible for any damage, theft, or loss of my equipment or property.
• I agree to maintain active professional liability insurance at all times while operating under or affiliated with Ur Massage Therapist events or referrals.
• I commit to maintaining professional conduct in all interactions with clients, venues, event coordinators, and other professionals.
• I acknowledge that I am responsible for following all local, state, and federal regulations, licensing requirements, and any laws governing massage therapy within the areas I serve.
• I understand and agree that Ur Massage Therapist functions solely as a booking and referral platform and is not liable for any actions, disputes, or claims arising from services I provide.
• I acknowledge that all clients and bookings I obtain through Ur Massage Therapist remain clients of the platform, and I agree to communicate and handle all event-related questions and concerns professionally and with integrity.
• By submitting this application, I confirm that I have read, understood, and agree to the terms outlined above.
